Patients are asked to bring in their Insurance Card(s) Information for all visits. 

20               Holiday:  President’s Day

7,14,21 & 28      Immunizations Clinic, 1:00-3:30p.m. every Tuesday - Adult Health, Blood Pressure Screenings & Flu shots: New patients please bring your immunization records. (Free or minimal charge for Vaccines depending on Program Eligibility)

*Beginning the 2012-2013 School year West Virginia Law requires Tdap booster &Meningococcal/ Meningitis for students entering 7th & 12th grades. *A second meningitis (booster) is recommended for 16-18 year olds if only one dose of Meningitis was given between the ages of 11-15. *We encourage all 11 year olds to get Gardasil/HPV/Human Papillomavirus Vaccine, and Varicella/ chickenpox booster.

7      Lab Testing Clinic (By appointment ONLY) will be on the 1st Tuesday of each month. Diabetes, Cholesterol, etc., - minimal charge

1 & 15      Family Planning Clinics – Free or reduced rates on pap smears with Tamara Kessel, CNM
13          HealthCheck/Pediatric Clinic - with Gina L Sizamore, PA-C.

14        Family Planning Supplies

9 and 23     BCCSP (Breast and Cervical Cancer Screening Program)  Free or reduced rates on Pap Smears with Sandria L. Glasscock, RN
